It protects against Leptospira bacteria, which dogs typically pick up from water, soil, or mud contaminated with infected wildlife or rodent urine — puddles, ponds, and rivers are common exposure spots. Leptospirosis can cause serious kidney and liver damage in dogs, and it's zoonotic, meaning it can spread from an infected dog to people too.

What is a "lepto 4-way" vaccine?

Leptospira bacteria come in several different strains (serovars), and early lepto vaccines only covered two of them. A "4-way" (or "L4") vaccine covers four of the most common serovars instead of two, which is why it's become the more commonly recommended version — ask your vet whether the lepto vaccine they use is a 4-way formulation.

AAHA classifies leptospirosis as a "non-core" vaccine — meaning it's recommended based on lifestyle and regional risk rather than required for every dog — but AAHA's own guidelines note that lepto risk is broader than once thought, and many vets now recommend it for most dogs regardless of whether they're rural or urban, since wildlife (and the puddles they leave behind) show up in city parks and suburban yards too.
